Admission Criteria:

1. Medicaid ID Waiver

2. Male or Female

3. 18 years or older

4. Must have a dignosis of intellectual disability *

5. Must need 24 hour supervision and assistance in order to be successful in living in the community. 

6. Resident of Virginia

Discharge Criteria:

1. Resident has developed such a level of independence that 24 hour supervision is no longer required. 

2. Resident, family, or placing agency makes a discharge decision.

3. Resident no longer meets admission criteria.

4. Resident is no longer compliant with rules of home

Exclusion Criteria:

1. Needs of individual cannot be met by Raven's Nest. 

2. Staff cannot meet the medical needs of resident.
